Job Description:

Reports to:      Director of Veteran Services          

Employee Type:         Full-Time, Monday through Friday

Location:       Wall Township, New Jersey

Salary:            $75,000.00

 

Job Summary:

The Case Manager position is responsible for the development and case management of a new community-based Veteran Resource Program focused on providing services for at-risk New Jersey Veterans. This position is responsible for the day-to-day operations of the program. The Case Manager is involved in every aspect of the program, including participant service delivery, case management, case follow-up, quality control and fidelity to program mission, collaboration with local, state, and federal agencies, development of Veteran community partnerships, and monitoring and reporting on program outcomes.

This is a grant-funded position. Continued employment is dependent upon grant funding availability.

 

Supervisory Relationships: The Case Manager reports directly to the Director of Veteran Services.

 

Principal Duties & Responsibilities:

  • Develop and manage day-to-day activities associated with the Veteran Resource Program.
  • Provide case management and ongoing linkage to community supports.
  • Serve as program liaison with Veteran service organizations and networks.
  • Conduct and engage in Veteran community outreach/engagement
  • Collaborate with local, state, and federal agencies to identify potential Veteran Resource participants and assess participant needs.
  • Develop collaborative relationships to provide participants with opportunities for housing, transportation, social services and medical/behavioral health services.
  • Ensure an up-to-date list of all community partners is maintained.
  • Develop and maintain a Community Resource
  • Perform other duties as assigned

 

Education, Knowledge, Skills and Abilities:

Bachelor or Associate Degree. High school degree or GED plus work/life experience also accepted in lieu of degree.

Knowledge and experience with veteran services preferred.

Strong organization and communication skills required. Must possess a valid Driver’s License. Knowledge of Microsoft Office applications including word processing and spreadsheet software.
